29 CFR 1926.852(a): Material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected. On the south side of commercial building, the employer did not provide a protective barrier around where demoed debris were being thrown down the chute to the ground level exposing employees to struck-by hazards.

29 CFR 1926.852(c): A substantial gate shall be installed in each chute at or near the discharge end. A competent employee shall be assigned to control the operation on the gate and the backing and loading of trucks. On the south side of commercial building, the employer did not provide a protective gate to prevent entering inside the chute area where debris were being thrown down exposing employees to struck-by hazards.

29 CFR 1926.852(e): Chute openings, into which workmen dumped debris, were not protected by substantial guardrails approximately forty-two inches above the floor or other surface on which men stood to dump material. On the fourth floor of a commercial building, the employer did not protect employees from fall hazards of approximately 32 feet at the wall opening of a demolition chute.
