29 CFR 1926.451(c)(1)(i): Guys, ties, and braces were not installed at locations where horixontal members support both inner and outer legs.  On 9/6/16, at the 39 E. May Street, Winder job site, employees were working on scaffolds that were not constructed or supported with cross braces at the inner and outer legs leaving the scaffold system unstable.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access.   On or about 9/6/16, at the 39 East May Street, Winder job site, employees were permitted to climb the cross braces and scaffold frames themselves instead of being provided and required to use a ladder. Scaffolds used at the site were three frames tall.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On or about 9/6/16, at the 39 East May Street, Winder job site, the employer did not have a competent person inispect scaffolds before permitting employees to work on those scaffolds. Scaffold deficiencies included: no fall protection, cross braces, ladders for access or full decking.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  On or about 9/6/16, at the 39 East May Street, Winder job site, employees were permitted to work from scaffolds that were not equipped with guardrails. Employees worked at levels of 12 to 18 feet above lower levels without fall protection.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On 9/6/16, the employer did not develop and implement a program to provide training to employees to enable them to recognize and eliminate hazards associated with scaffold work.
