29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems. On or about 1 September 2016, at this commercial construction site, employees were exposed to a fall hazard while performing roofing activities without proper fall protection at a height greater than 6 feet from the ground level.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways structure. On 1 September 2016, at this commercial construction site, employees worked without fall protection while using a ladder to access a roof from a scaffold.
