29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At Southeast corner (front) of building: On or about September 6, 2016 and at times prior the employer exposed his employees to fall hazards in that employees were allowed to access a roof that was 19 feet above the ground with a ladder that was set up where the ladders side rails did not extend at least 3 feet above the upper landing surface.
