This employee was not protected by a personal fall arrest system (PFAS) and/or other fall protection device.     In the Alternative:    29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    a.) Fitness Erie, Inc. dba Pennbriar Tennis & Health, 100 Penn Briar Drive, Erie, PA 16509, roofs of structures, Northwest section of the Fitness building roof/Southwest section of the Tennis building roof, on or about September 6th, 2016 and times prior thereto:  An employee was working on the Fitness building and Tennis Court roof structures and was exposed to a fall hazard of approximately 18.2 feet to the ground below.
