29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:          a.) Vogt Finishes, LLC, 3209 Pennsylvania Ave, Erie, Pa 16504, roof of structure, Southwest corner of the First Good Samaritan AME Church, on or about September 6th, 2016 and times prior thereto:  Two employees were performing priming/painting operations on the roof of the First Good Samaritan AME Church and were exposed to fall hazards of approximately 9.4 feet to the ground below.  The employees were not utilizing fall protection during work operations on the roof.

Vogt Finishes, LLC, 3209 Pennsylvania Ave, Erie, Pa 16504, roof of structure, Southwest corner of the First Good Samaritan AME Church, on or about September 6th, 2016 and times prior thereto:  Employees worked with and around hazardous substances including, but not limited to,  Sherman Williams Kem Bond HS B50 WZ-4 Primer during work operations on the roof of the facility.  The employer did not develop and implement a written Hazard Communication Program.
