29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    (a) On or about August 25, 2016 for the employee engaged in pipeline completion operations that was struck by a Landoll 1550 In-Row Ripper jack stand where the employer failed to verify that the equipment in use was in accordance with the manufacturers specifications vicinity of County Highway 100 and 60th Street Tioga, ND.         NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to inspect the Landoll 1550 In-Row Ripper and to train the employees in accordance with the manufacturers operators manual.
