29 CFR 1926.502(i)(3): All floor hole covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: a) Army Reserve Center, Ashley, PA: The plywood cover over a floor hole near the north wall that was created when a large duct was removed was not secured to prevent displacement exposing workers to a 13 foot fall, on or about September 7, 2016. b) Army Reserve Center, Ashley, PA: The covers over four floor holes outside of the east stairwell were not secured to prevent displacement exposing workers to a fall hazard of as much as 13 feet, on or about September 7, 2016. ABATED DURING INSPECTION CERTIFICATION NOT REQUIRED
