29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: Instance a) On or about September 22, 2016, at the West Point Dairy Products, LLC, facility, the employer did not document that it complied with recognized and generally accepted good engineering practice (RAGAGEP), such as ASHRAE 15 (2013), Section 8.13(f), when it did not ensure that three south wall penetrations for piping and conduit were sealed to prevent ammonia from communicating from the ammonia engine/compressor room to occupied spaces, exposing employees to the hazards of inhalation of toxic ammonia and ammonia burns.

C) Pressure relief capacity for surge drum SD-S-3 is not sized for the capacity of the surge drum plus connected Silo #3. D) Pressure relief capacity for surge drum SD-S-4 is not sized for the capacity of the surge drum plus connected Silo #4. E) Pressure relief capacity for surge drum SD-S-5 is not sized for the capacity of the surge drum plus connected Silo #5.
