29 CFR 1910.217(b)(7)(x): Foot operated tripping control(s) used mechanical power press(es) with part revolution clutches were not protected to prevent operation from falling or moving objects, or from unintended operation by accidental stepping onto the   (a) Shop - On or about June 15, 2016, the employer did not ensure that the 40 Ton Punch Press Johnson 3 (Machine #69) with a foot operated tripping control was protected by a guard to prevent the unintended operation.  No abatement certification or documentation is required for this item.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHAs public web site at www.osha.gov:    (a) The employer did not report a amputation that occurred on June 15, 2016 as result of a work-related incident to OSHA within twenty-four (24) hours.  No abatement certification or documentation is required for this item.
