29 CFR 1910.28(b)(1)(i): The employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a guardrail system.   On or about September 7, 2016: The maintenance employee working at the facility located at 775 Hwy 42 North is exposed to approximately 10 feet high fall hazards where he changes filters on the HVAC system located on top of the gun ranges.

On or about September 7, 2016: Employees working as range safety officers (RSOs) are exposed to noise at or above 85 decibels as an 8-hour time weighted average (TWA).  The employer did not establish a valid baseline audiogram for the employees.

On or about September 7, 2016:  Employees working as range safety officers (RSOs) or performing clean-up of the range are exposed to lead.  The employee did not ensure that the ventilation system was performing properly by conducting periodic measurements of the system.   OTS Capital Partners, LLC was previously cited for a violation of this occupational safety and health standard 1910.1025(e)(4)(I), which was contained in OSHA inspection number 988000, citation number 2, item number 1 and was affirmed as a final order on October 14, 2014, with respect to a workplace located at 775 Hwy 42 North, McDonough, Ga.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:     On or about September 7, 2016:   Employees working at the facility located at 775 Hwy 42 North, McDonough, Georgia are exposed to lead.  The employer did not ensure all workplace surfaces were as free as practicable of the accumulation of lead.  Wipe samples were collected on September 7, 2016 with the following results:    a.  Target counter in the range corridor area - 209.100 micrograms per 100 square centimeters    b.  Desk/work area near the register and computer - 76.860 micrograms per 100 square centimeters c.  Trophy cabinet in the range corridor area - 50.190 micrograms per 100 square centimeters d.  Wooden countertop to the right of the entrance door - 11.880 micrograms per 100 square centimeters   OTS Capital Partners, LLC was previously cited for a violation of this occupational safety and health standard 1910.1025(h)(1), which was contained in OSHA inspection number 988000, citation number 1, item number 5 and was affirmed as a final order on October 14, 2014, with respect to a workplace located at 775 Hwy 42 North, McDonough, Ga.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.   On or about September 7, 2016: The employees working at the are exposed to chemicals such as but not limited to Outers Nitro Solvent Bore Cleaner, Crest Ultrasonics Chem-Crest 400-L, a solution used in the ultrasonic gun cleaner, which is a skin and eye irritant, Kobolt Air Compressor Oil, Isopropyl alcohol, D-Lead All Purpose Cleaner, and Goo Gone All Purpose Cleaner.  The employer did not maintain or have a written hazard communication program.  OTS Capital Partners, LLC was previously cited for a violation of this occupational safety and health standard 1910.1200(e)(1), which was contained in OSHA inspection number 988000, citation number 1, item number 7 and was affirmed as a final order on October 14, 2014, with respect to a workplace located at 775 Hwy 42 North, McDonough, Ga.

On or about September 7, 2016: The employees working within the facility located at 775 Hwy 42 North, McDonough, Georgia are exposed to fire hazards.  The employer did not ensure that the fire extinguishers were serviced annually. During the inspection it was observed that all of the fire extinguishers were last serviced November 6, 2013.
