29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    A. Saavedra & Sons Home Improvement, LLC worksite located at 5811 Waterville Monclova Rd., Waterville, Ohio: On or about September 7, 2016, the employer did not ensure employees were provided with proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Three employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     A. Saavedra & Sons Home Improvement, LLC worksite located at 5811 Waterville Monclova Rd., Waterville, Ohio: On or about September 7, 2016, the employer did not ensure the side rails of the extension ladders were extended at least three feet above the landing surface. Five employees were exposed to falls while using the ladder to access the roof.

Saavedra & Sons Home Improvement, LLC worksite located at 5811 Waterville Monclova Rd., Waterville, Ohio: On or about September 7, 2016, the employer did not have a plan or program to identify jobsite hazards which assist employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as, but not limited to fall hazards, ladder use, and personal protective equipment.  Saavedra & Sons Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1) which was contained in OSHA inspection number 1089707, citation number 1, item number 1a and was affirmed as a final order on October 21, 2015, with respect to a workplace located at Maumee, Ohio.

Saavedra & Sons Home Improvement, LLC worksite located at 5811 Waterville Monclova Rd., Waterville, Ohio: On or about September 7, 2016, the employer did not ensure employees were protected from falls while installing weatherproofing on the residential structure. The employees were exposed to a fall distance of approximately 10.00 feet from the 10/12 pitch roof.   Saavedra & Sons Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1089707, citation number 1, item number 1b and was affirmed as a final order on October 21, 2015, with respect to a workplace located at Maumee, Ohio.
