During the assembly and disassembly of a DEMAG AC 500-1 Telescoping Boom Crane (serial number 84090), the operator was not informed when employees accessed areas on the crane that were out of the operator's view exposing employees to struck-by and crushed by hazards.

29 CFR 1926.1403(a): Manufacturer procedures applicable to assembly and disassembly were not complied with: While assembling and disassembling a DEMAG AC 500-1 Telescoping Boom Crane (Serial Number 84090), the employer failed to comply with manufacturer prohibitions and procedures by: a) allowing employees to walk on the main boom and; b) allowing employees other than the operator to remain on the crane during the addition or removal of the counterweights. This exposed employees to fall hazards, struck-by hazards, and crushed-by hazards.

During assembly and disassembly of a DEMAG AC 500-1 Telescoping Boom Crane (serial number 84090), employees were exposed to struck-by and crushed-by hazards while standing on and near the counterweight tray while counterweights were being hoisted and lowered.
