29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) Rubi Stone, Inc. at 3395 E. Virginia Ave., Denver, CO 80209; On and before September 7, 2016, the exposing employer failed to ensure that the platforms on all working levels of scaffolds were fully planked. The employer did not fully plank the top level and the level directly below it. Condition exposed employees working on those levels to fall hazards of approximately 30 feet while conducting stucco and stonework off a tubular scaffold system.

Condition exposed employees performing stucco work to fall hazards due to the distance between their work platform and the face being greater than 18 inches.

29 CFR 1926.451(g)(4): Guardrail systems on the scaffold were not installed to meet the requirements of this section and did not meet the provisions as stipulated by Appendix A to this subpart: (a) Rubi Stone, Inc. at 3395 E. Virginia Ave., Denver, CO 80209: On and before September 7, 2016, the exposing employer did not ensure that guard rails installed on his scaffold system met OSHA guidelines. Specifically, a working deck on the scaffold at a height of approximately 30 feet did not have any mid rails, and the working deck below it at a height of approximately 24 feet did not have any top or mid rails. Condition exposed employees working at those heights to fall hazards of approximately 24 feet.
