29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a. Detailing Area: On or about September 7, 2016, and at times prior, employees were exposed to respiratory hazards from being required to wear a Sperian half mask respirator with combination cartridges without having a medical evaluation prior to use.   b. Detailing Area: On or about September 7, 2016, and at times prior, employees were exposed to respiratory hazards from being required to wear a Sperian half mask respirator with combination cartridges without having been fit tested prior to use.   c. Detailing Area: On or about September 7, 2016, and at times prior, employees were exposed to respiratory hazards from being required to wear a Sperian half mask respirator with combination cartridges and N95 dust masks without having been given adequate information and training on how to care for and maintain respirators.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a. Detailing Area: On or about September 7, 2016, and at times prior, employees were exposed to chemical burns from corrosive materials including, but not limited to, First Place Finish Car Care System Tough Enough and First Place Finish Car Care System Hawaiian Super Suds Hyper without an eye wash or emergency shower within the immediate vicinity.   b. Lube Area:  On or about September 7, 2016, and at times prior, employees were exposed to chemical burns from corrosive materials including, but not limited to, Sodium Hydroxide, without an eye wash within the immediate vicinity.

Shop and Detailers Area: On or about September 7, 2016, and at times prior, employees were exposed to chemical hazards due to a lack of adequate training on hazardous corrosive chemicals found in the workplace.  b. Shop and Detailers Area: On or about September 7, 2016, and at times prior, employees were exposed to chemical hazards due to lack of labeling including, but not limited to Dex Cool Coolant, Oil, and a bottle marked degreaser.
