29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a- Side of Building's Entrance, 301 Essex Avenue, Lakewood NJ: Company employees, not wearing protective helmets, was exposed to struck by hazards when performing red brick installation to the exterior surface of a commercial building, and other masonry related activities, from the fourth buck of a prefabricated, tubular welded framed. Condition observed on or about 09/08/16.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a- Side of Building's Entrance, 301 Essex Avenue, Lakewood NJ: Company employees, not supplied with fall protection, was exposed to fall hazards in excess of 19 feet to the ground surface below when performing red brick installation to the exterior surface of a commercial building, and other masonry related activities, from the fourth buck of a prefabricated, tubular welded framed scaffold that was missing guardrails, mid-rails, and end-stops. Condition observed on or about 09/08/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
