A custodial employee is potentially exposed to hazardous chemicals, including but not limited it the following:   1) De-Sovl-It  2) Lysol Toilet Bowl Cleaner  3) Pine Sol Cleaner  4) Lysol Power and Fresh Multi-Surface Cleaner  5) HDX Foaming Cleaner    b) Capitol Affiliated LLC at 573 W. Crete Circle #104, Grand Junction, CO 81505:  On and before September 6, 2016, the employer did not ensure that any employee using a respirator voluntarily was provided with the information contained in Appendix D to this section when the employer determined that any voluntary respirator use was permissible. This employer provided SAS Safety Nuisance Dust Masks (Model 2985) for voluntary use. This condition exposed the employees to a respiratory hazard.

Crete Circle #104, Grand Junction, CO 81505: On and before September 6, 2016, the employer did not select and use a NIOSH-certified respirator in compliance with the condition of its certification. Employees were provided non-NIOSH certified nuisance dust masks, SAS Safety Model 2985, for non-mandatory usage throughout the facility. The use of a non-certified NIOSH respirator potentially creates a hazard to the employee because the nuisance dust mask may not be capable of providing appropriate respiratory protection.

Crete Circle #104, Grand Junction, CO 81505:  On and before September 6, 2016, the employer did not ensure that employees were provided with information and training necessary to protect them from spills or leaks of a hazardous chemical in a sealed container. The employees in the receiving area were required to unseal, clean, and salvage damaged and leaking containers with potentially hazardous chemicals.  This condition exposed employees to a chemical hazard.
