29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    In Terminal A, Gate 39, third floor:  The employer did not ensure employees working on or around an elevator shaft were protected from a fall hazard such as but not limited to when the guardrails were not installed correctly or while being repaired.  Employees were exposed to an unprotected fall hazard of approximately 30 feet to the concrete below.
