In the printing shop, employees were exposed to amputation and caught in hazards, in that, there was no control of hazardous energy program in place while conducting maintenance on machinery such as but not limited to, the Mitsubishi 6 color diamond 2008, NBO American B30 Perfection 444/3, NB26444/C and the Heidelberg SM52 SN# 205286, on or about 09/08/2016.
