29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) On or about 09/07/16, job site located at 182-23 80th Drive. Jamaica, NY  Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and material from the employees who working 14-feet overhead on a tubular welded frame scaffold. The employees were not provided with protective helmets.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)  A).       On or about 09/07/16,  job site located at 182-23 80th Drive, Jamaica, NY    Employees applying stucco to exterior wall of a 2-story house while on tubular welded frame scaffolds were exposed to the hazard of falling approximately 14 ft. to the ground. The scaffold platforms were not fully planked.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  A).      On or about 09/07/16,  job site located at 182-23 80th Drive, Jamaica, NY    Employees applying stucco to the exterior of a 2-story house while on a tubular welded frame scaffold were exposed to the hazard of falling approximately 14 ft. to the ground.   Guardrails or personal fall arrest systems were not provided.    ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

Jamaica, NY  An employer did not develop a written hazard communication program for the site. Employees mixing stucco were exposed to a hazardous chemicals, such as, but not limited to, cement which contains silica.  ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
