29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:    a) 153 Main Street, South Bound Brook, NJ 08880, North Side at Unit #206. The employer allowed his employees to secure plywood sheathing to structure when operating pneumatic nail guns while not wearing any eye and or face protection exposing the employees to eye and face injuries from flying debris.     b) 153 Main Street, South Bound Brook, NJ 08880, North Side at Unit #201.The employer allowed his employees to operate a circular saw to cut plywood sheathing while not wearing any eye and or face protection exposing the employees to eye and face injuries from flying debris.     Occurred on or about 08/31/16.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a) 153 Main Street, South Bound Brook, NJ 08880. The employer allowed his employee to operate a circular saw to cut plywood sheathing when the extension cord being used had a damaged strain relief exposing the employee to electrical shock hazards.      Occurred on or about 08/31/16

The employer allowed his employees to carry plywood sheathing up an aluminum extension ladder exposing the employee to fall hazards if the employee was to lose his balance and fall.   b) 153 Main Street, South Bound Brook, NJ 08880, North Side at Unit #210. The employer allowed his employee to carry plywood sheathing up an aluminum extension ladder exposing the employee to fall hazards if the employee was to lose his balance and fall.   c) 153 Main Street, South Bound Brook, NJ 08880, North Side at Unit #201.The employer allowed his employee to carry plywood sheathing up an aluminum extension ladder exposing the employee to fall hazards if the employee was to lose his balance and fall.
