29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   On  February 5 2016, Elmer Miller dba Miller Building Systems, LLC, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:        Citation 1 Item 1  The Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 945843, citation number 2, item number 1 and was affirmed as a final order on April 21, 2014, with respect to a workplace located at 312 Banbury Lane, Savoy, IL.  The Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 967007, citation number 2, item number 1 and was affirmed as a final order on December 5, 2014, with respect to a workplace located at 3906 Patriot Lane, Savoy, IL.  The Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 1114206, citation number 4, item number 1 and was affirmed as a final order on May 26, 2016, with respect to a workplace located at Corner of Finch Dr. and Country Ridge Dr., Mahomet, IL.  The Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 1154396, citation number 3, item number 1 and was affirmed as a final order on December 7, 2016, with respect to a workplace located at 605 Country Ridge Dr., Mahomet, IL.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about September 7, 2016, Elmer Miller dba Miller Building Systems, LLC does not require the use of any fall protection during residential construction work.  This violation was most recently observed when employees were exposed to fall hazards of approximately 10 ft. without fall protection, on a residential construction site in Champaign, Illinois.   Elmer Miller dba  Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 831261, citation number 2, item number 3 and was affirmed as a final order on April 3, 2013, with respect to a workplace located 303 Banbury Lane, Savoy, IL 61874.  Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 945843, citation number 1, item number 3 and was affirmed as a final order on February 18, 2014, with respect to a workplace located at 312 Banbury Lane, Savoy, IL 61874.  Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 967007, citation number 1, item number 2 and was affirmed as a final order on August 26, 2014, with respect to a workplace located at 3906 Patriot Lane, Savoy, IL 61874. Elmer Miller dba Miller Building Systems, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number: 1138771, citation number 1, item number 2 and was affirmed as a final order on July 5, 2016, with respect to a workplace located at 4808 Oakdale Drive, Champaign, IL 61822.
