29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections: On or about August 25, 2016, C.H.I. Overhead Doors, Inc. had employees exposed to physical hazards such as broken bones and/or amputation, due to the daily use of overhead cranes in the Stamping Department and Rolling Steel, and the employer did not conduct frequent crane inspections. These inspections would have included the following: a. All functional operating mechanisms for maladjustment interfering with proper operation Daily, b. Deterioration or leaking in lines, tanks, valves, drain pumps, and other parts of air or hydraulic systems Daily, c. Hooks: Daily visual inspections, monthly inspection with certification record; d. Hoist chains: Daily visual inspections, monthly inspection with certification record; e. All functional operating mechanisms for excessive wear of components Daily; f. Rope reeving for noncompliance with manufactures recommendations Daily.

29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section: On or about August 25, 2016, C.H.I. Overhead Doors, Inc. had employees exposed to physical hazards such as broken bones and/or amputations, due to the daily use of overhead cranes in the Stamping Department and Rolling Steel, and the employer did not conduct periodic crane inspections. These inspections would have included the following: a. Deformed, cracked, or corroded members, b. Loose bolts or rivets, c. Cracked or worn sheaves and drums, d. Worn, cracked or distorted parts such as pins, bearings, shafts, gears, rollers, locking and clamping devices, e. Excessive wear on brake system parts, linings, pawls, and ratchets, f. Load, wind, and other indicators over their full range, for any significant inaccuracies, g. Gasoline, diesel, electric, or other powerplants for improper performance or noncompliance with applicable safety requirements, h. Excessive wear of chain drive sprockets and excessive chain stretch, i. Electrical apparatus, for signs of pitting or any deterioration of controlled contactors, limit switches and pushbutton stations.
