Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  a. 29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances shall be of three-wire type and shall be designed for hard or extra-hard usage. Flexible cords used with temporary and portable lights shall be designed for hard or extra-hard usage. NOTE: The National Electrical Code, ANSI/NFPA 70, in Article 400, Table 400-4, lists various types of flexible cords, some of which are noted as being designed for hard or extra-hard usage. Examples of these types of flexible cords include hard service cord (types S, ST, SO, STO) and junior hard service cord (types SJ, SJO, SJT, SJTO).  a. On September 9, 2016, the employees were using an extension cord of the two wire type that was rated for junior hard service, thereby exposing employees to an electrical hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  a. On September 9, 2016, an employee was working from an aerial lift at a height greater than 15 feet to perform framing activities without being tied off, thereby exposing the employees to a fall hazard.
