29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances, severe cuts or lacerations, severe abrasions, punctures, chemical burns, thermal burns, and harmful temperature extremes.    a)   O'Charley's, Kitchen, Prattville, AL:  On or about September 7, 2016 and at times prior, the employer exposed employees to laceration and amputation hazards in that employees were not required to use hand protection while using knives for food preparation in the kitchen.

29 CFR 1904.39(a)(2): The in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident was not reported within 24 hours.  a)   O'Charley's, Kitchen, Prattville, AL:  On or about September 7, 2016 and at times prior, the employer failed to timely report an employee amputation that occurred after an employee cut his finger while cutting onions in the kitchen.
