29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 850 N. Maxwell St.  Allentown, PA, Roof: On or about September 2, 2016, employees were without any fall protection and working on the roof of a 1.5 story residential home, exposing employees to a fall hazard of no less than 15 feet.   A-1 Lehigh Valley Roofing Co., Inc.  was previously cited for a violation of its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 936345, citation number 1, item number 1 and was affirmed as a final order on 11/1/2013, with respect to a workplace located at 1440 Lehigh Street, Whitehall, PA 18052.    A-1 Lehigh Valley Roofing Co., Inc.  was previously cited for a violation of its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 907997, citation number 1, item number 1 and was affirmed as a final order on 9/26/2013, with respect to a workplace located at 1330 Hamilton Street, Allentown, PA 18101.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 850 N. Maxwell St.  Allentown, PA, Roof: On or about September 2, 2016, employees used an aluminum extension ladder to access a residential dwelling, but the rungs did not extend at least 3 feet above the upper landing surface, exposing employees to a fall hazard of no less than 15 feet.  A-1 Lehigh Valley Roofing Company, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 936345, citation number 1, item number 2 and was affirmed as a final order on 10/17/13, with respect to a workplace located at 4140 Lehigh Street, Whitehall, PA. 18052.
