Three sides of the scaffold had guardrails. The scaffold was approximately 22" from the face of the building and had no inside guardrail. The employee was exposed to an approximately 30 ft. fall;on or about 9/12/16.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
