29 CFR 1926.502(d)(1): Connectors were not drop forged, pressed or formed steel, and/or made of equivalent materials: a)1011 Riverdale St., West Springfield, MA: On or about August 18, 2016, an employee working from a raised aerial lift was attached to the basket with a rope which was knotted to both his full body harness and the attachment point on the basket instead of using an appropriate lanyard with drop forged, formed steel connectors or their equivalent.
