29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about September 8, 2016, at Lot 63, 599 Loretta Way, Grayson, Georgia: An employee performing framing work was exposed to struck-by hazards from falling nail guns, plywood, and other materials of the employees working from the 2nd floor above him as he worked from the ground without using head protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about September 8, 2016, at Lot 63, 599 Loretta Way, Grayson, Georgia: a) Six employees performing framing work were exposed to a fall hazard of approximately 13.3-feet to 20.8-feet when they worked at the unprotected edges of a house without using fall protection b) Six employees performing framing work were exposed to a fall hazard of approximately 11.1-feet and greater when they worked within six feet of an unprotected floor opening which was at least six feet above a lower level without using fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about September 8-9, 2016, at Lot 63, 599 Loretta Way, Grayson, Georgia: Six employees performing framing work were exposed to a fall hazard of approximately 15.6-feet when they used a Louisville aluminum extension ladder that was not set three feet above the 2nd floor landing.
