29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the west side of the commercial building, employees engaged in roofing activities on a 6:12 pitch roof without fall protection were exposed to a fall hazard of approximately 19 feet to the concrete surface below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On the west side of the commercial building, employees used a ladder to access and egress the roof that did not extend at least 3 feet above the upper landing surface were exposed to a fall hazard.
