29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:    a)  At the jobsite, on or about 9/8/16:  The employer did not ensure that anchor points for personal fall arrest systems were properly installed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  At the jobsite, on or about 9/8/16:  The employer did not ensure that employees working on an 10:12 and a 4:12 pitch roof were protected from falling 20 feet and 11 feet (respectively) to ground.         The Roofing Guys was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 315849273, citation number 1, item number 1a and was affirmed as a final order on 4/25/12, with respect to a workplace located at 129 Iron Oak Circle, North Syracuse, NY.    The Roofing Guys was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 942969, citation number 1, item number 1 and was affirmed as a final order on 8/18/14, with respect to a workplace located at 4436 Ashfield Terrace, Syracuse, NY.    The Roofing Guys was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 921490, citation number 2, item number 1 and was affirmed as a final order on 10/17/14, with respect to a workplace located at 1123 Cold Springs Road, Liverpool, NY.    The Roofing Guys was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 983577, citation number 2, item number 1a and was affirmed as a final order on 7/6/15, with respect to a workplace located at 3622 Southeast Townline Road, Marcellus, NY.
