29 CFR 1926.305(d)(1)(i): The load was not cribbed, blocked, or otherwise secured at once after the load had been raised: Location: 90 Fawcett Street, Cambridge, MA On or about 9/8/16, the employer did not ensure that the beam and Ellis bridge jack, model BJ-21 that was under tension and on or about 6-inches off of the ground was blocked or secured to prevent the beam and jack from moving. An employee went to remove the last bolt connected to the Ellis jack and beam, when the other beam shifted, struck, and broke the employee's right leg.
