29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Building 2, Cutting Area:  1) Pinchpoints created by the rolling machine and the material being cut were not guarded.  2) The shear was not adequately guarded.  3) The control lever of the rolling machine was not protected against accidental movement.   4) The foot pedal of the shear was not covered to prevent accidental activation.   A worker was injured when his hip struck the control lever and the material brought the worker's hand into the rolling machine where his arm was crushed.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  Worksite:  The employer did not report to OSHA an accident which resulted in the hospitalization of an employee.
