29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  On or about September 13, 2016, at the above addressed worksite, the employer did not administer a continuing, effective hearing conservation program whenever employee noise exposures equaled or exceeded the 8-hour time-weighted average action level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent. The following monitored employees exceeded the action level (all exposure calculations included a zero increment for the time not sampled):    Sandblaster Operator: exposed to continuous noise of 91.1 dBA or the equivalent noise dose of 117.4% during the 413 minute sampling period on January 19, 2017.

29 CFR 1910.1000(c): An employee(s) was exposed to chemical(s) in excess of the 8-hour time weighted average limits listed in Table Z-3:      On or about January 19, 2017, at the above addressed worksite, the following monitored employees were overexposed to dust containing Silica in excess of the PEL (all calculations assumed zero exposure for the unsampled period of time):    a) Sandblaster Operator: exposed to Silica, Crystalline Quartz (Respirable Fraction) at an 8-hour time-weighted average of 13 milligrams per cubic meter of air, approximately 102 times the calculated PEL of 0.13 milligrams per cubic meter of air. Sampling was performed for 315 minutes on January 19, 2017.  b) Sandblaster Operator: exposed to Silica (Quartz, Total) at an 8-hour time-weighted average of 15.19 milligrams per cubic meter of air, approximately 41 times the calculated PEL of 0.37 milligrams per cubic meter of air. Sampling was performed for 142 minutes on January 19, 2017.
