Employees on this site were exposed to hazards including, but not limited to fall and struck by hazards.  Condition noted on or about 09/13/16.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Condition observed on or about 09/13/16. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a- 76 Liverpool Way, Medford NJ: Employees, not properly utilizing the fall protection at the worksite as specified by the Standard and the equipment manufacturer's requirements, were exposed to fall hazards in excess of 11 feet to the ground surface below when performing roof shingle installation and other related activities from a residential structure's roof with a pitch of approximately 6 on 12.  Condition observed on or about 09/13/16. 29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    a- 76 Liverpool Way, Medford NJ: Employees, not properly utilizing the fall protection at the worksite as specified by the Standard and the equipment manufacturer's requirements, were exposed to fall hazards in excess of 11 feet to the ground surface below when performing roof shingle installation and other related activities from a residential structure's roof with a pitch of approximately 6 on 12.  These employees had not received fall hazard recognition training from a competent person at the time of the worksite inspection.  Condition noted on or about 09/13/16.
