29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) R.P.S. Construction, LLC at 2525 18th Street, Denver, CO 80222: On or about August 31, 2016, the exposing employer did not ensure that an employee using a pneumatic nail gun while siding wore safety glasses. The employee was using up to 2 1/4 inch siding nails with a nail gun to nail hardie board without the use of safety glasses. This condition exposed this employee to eye injuries from the hazard of projectiles to the eyes.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: (a) R.P.S. Construction, LLC at 2525 18th Street, Denver, CO 80222: On or about August 31, 2016, the exposing employer allowed the use of worn and frayed electrical cords. The cord running to a portable Skilsaw model SP177M, serial number FQ1277, had the outer insulation pulled from the plug. The yellow extension cord running to this same Skilsaw also had the outer insulation pulled from its plug. The cord was set on a piece of siding above some water. The circuit was on a ground fault circuit interrupter (GFCI). Condition exposed employees to electrical shock hazards.
