At the construction site located at Building #1, employees were assembling a FRACO Act-4000, S/N 44060008, Mast Climber Work Platform, Unit -2. An employee's hand was pinned between the lifting platform and the mast section, when the mast climber system became active, during the manual installation of anchoring devices, on or about 09/03/2016. No inspection(s) had been conducted to identify damage to the system's remote control cable, on or about, 09/13/2016.

Tamiami Trail, Bonita Springs (FL), employees were assembling a FRACO Act-4000, Mast Climber Work Platform. The outer jacket on the flexible cord attached to the mast climber's remote control system, was damage and/or had been repaired with electrical tape, on or about, 09/13/2016.
