29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    (a) Jobsite - 460 Angell Road, Lincoln, RI:  On or about 9/8/2016, an employee of another employer was allowed up on the single story low sloped roof and was not protected from the hazard of falling through two skylight holes cut into the roof, which were 52 inches by 52 inches wide with a fall of approximately 8-10 feet to the lower level, by personal fall arrest systems, cover(s), or guardrail systems.
