29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) plus 4 inches (10 cm) for every 10 kV over 50 kV was maintained:    a) At the South end of the Yard, on or about 9/13/2016: Employee was moving recyclable wood using a Komatsu excavator located on the wood pile underneath high voltage transmission lines and the employer did not ensure that the excavator will be operated in a way that will keep any part of the structure from entering the clearance. Employee was exposed to an electrical hazard.
