29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) On or about September 3, 2016, Osvaldo Tellez Alvarez, dba Walter's Construction, as the controlling employer, did not initiate and maintain a program which provided for frequent and regular inspections of the job site, materials and equipment by a competent person. Employees of Isai Salvador-Mauro, a subcontractor, were exposed to fall hazards of up to 13 feet while engaged in roofing activities on a two- story residential building with no fall protection in place.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  Osvaldo Tellez Alvarez, aka, Walter Construction, LTD.  was previously cited for violating occupational safety and health standard, 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 1186463, citation number 1, item number 1 and was affirmed as a final order on Dec. 19, 2016,  with respect to a workplace located at 3755 Tutt, Colorado Springs, CO 80922.  Osvaldo Tellez Alvarez, aka, Walter Construction, LTD. was previously cited for violating occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1113452, citation number 1, item number 6a and was affirmed as a final order on May 9, 2016, with respect to a workplace located at 14190 Temple Dr. Aurora, CO 80015.
