29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  At 2080 Berryhill Rd., Montgomery, Alabama:  On or about September 14, 2016 and at times prior; the employer failed to provide fall protection for employees involved in roofing activities on a high slope roof exposing his employees to a fall hazard of 12 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a)  At 2080 Berryhill Rd., Montgomery, Alabama:  On or about September 14, 2016 and at times prior; the employer allowed his employees to perform roofing activities without training them on fall protection to include but not limited to the requirement for fall protection, how to inspect a harness and how to install fall protection exposing his employees to a fall hazard of up to 12 feet  to the ground below.

29 CFR 1926.1053(a)(1)(ii): Portable ladders that were not self-supporting, without failure, at least four times the maximum intended load:  a)  At 2080 Berryhill Rd., Montgomery, Alabama:  On or about September 14, 2016 and at times prior;  the employee allowed his employees to use a job made ladder to gain access to the roof and the rungs of the ladder did not blocks installed exposing employees to a fall hazard of 12 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 2080 Berryhill Rd., Montgomery, Alabama:  On or about September 14, 2016 and at times prior; the employer failed to ensure a job made ladder used by employees to gain access to the roof had its rails extend at least three feet above the landing area exposing his employees to a fall hazard of 12 feet  to the ground below.
