29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit": On or about September 14, 2016, the employer failed to ensure that emergency exits in the main facility were identified by clearly visible and illuminated signage. Designated exits located on each end of the building and near the shipping department did not have the appropriate signage.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about September 14, 2016, the employer failed to ensure that an employer made device called a pump puller, which was being used by employees to pull the metal pumps off their support base and lower to the ground in order to repair or replace the pumps, was approved for use by the Powered Industrial Vehicle Manufacturer (Hyster/TCM) or an independent engineer, as safe to use as an attachment to the PIV.
