29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a: On or about September 14, 2016, at Southeastern Pipe and Precast, Inc. employees walking on semi-truck flat-bed trailers which had floorboards missing exposing large openings, exposing employees to fractures, broken bones, and fractures.

29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes set: a. On or about September 14, 2016 at Southeastern Pipe and Precast, Inc., parked inside the first bay on the east side, the employer did not ensure that employees operating a Caterpillar Forklift Model # GC55KSTR set the brakes before working more than 25 feet away, exposing employees to trip hazards. b. On or about September 14, 2016 at Southeastern Pipe and Precast, Inc., being used in front of the facility on the southeast side of the yard, the employer did not ensure that employees operating a Komatsu Forklift Model # FG70T-7 did fully lower the forks of the lift to the floor and did not set the brakes before working more than 25 feet away, exposing employees to trip and crushing hazards. c. On or about September 14, 2016 at Southeastern Pipe and Precast, Inc., parked in back of the facility on the southeast side of the yard, the employer did not ensure that employees operating a Komatsu Forklift Model # F080-6 did set the brakes before working more than 25 feet away, exposing employees to crushing hazards. d. On or about September 14, 2016 at Southeastern Pipe and Precast, Inc., parked in front of the facility on the southeast side of the yard, the employer did not ensure that employees operating a Taylor Forklift Model # TEB-160S did fully lower the forks of the lift to the floor, exposing employees to trip hazards.
