29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on September 12, 2016 on the jobsite located at 6633 Leesburg St, Lincoln, NE 68516.  Employees were observed removing roofing materials on a 8:12 pitched residential roof at heights greater than 6 feet above the ground.
