29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 09/07/2016, at the job site - an employee was pumping concrete inside the concrete block wall cavities while walking on top of the walls, exposing the employee to a 10 foot 3 inch fall hazard, in that, fall protection was not used.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.  a. At the job site - On September 07, 2016 an employee was hospitalized after falling and suffering a head injury. The employers transported the injured employee to the hospital, but did not report it within 24 hours.
