On or about September 14, 2016 at the facility located at 400 Gertrude St Kaukauna, WI 54130, the ladderway floor opening above the Wheelabrator Tumblast machine was not guarded by a standard railing and toeboards on all exposed sides to prevent an employee from walking directly into the opening.

On or about September 14, 2016 at the facility located at 400 Gertrude St Kaukauna, WI 54130, the employer did not provide a landing platform at the offset of the adjacent sections of the ladder used by the maintenance employee to access the area above the Wheelabrator Tumblast machine.

29 CFR 1910.27(b)(1)(iii): The clear length of rungs or cleats were not maintained at a minimum of 16 inches: a. On or about September 14, 2016 at the facility located at 400 Gertrude St Kaukauna, WI 54130, the employer did not ensure that the clear length of the first rung on the hinged ladder next to the Wheelabrator Tumblast machine was at least 16 inches. A piece of metal was attached to the rung to keep the ladder from moving while the maintenance employee accessed the area above the Wheelabrator Tumblast machine.
