29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 24, 2016, the employer did not ensure employees installing asphalt shingles on roof top of new residential housing were in use of fall protection, exposing the employees to a fall hazard of more than six (6) feet to the ground.

On or about August 24, 2016, the employer did not ensure the side rails of a ladder being used to access a roof extended at least three feet above the landing, exposing employees to a fall hazard.
