29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a) On or about 09/01/2016 - Dato department, building #3, employees performing maintenance and servicing on machinery such as, but not limited to, the Selco WNT 600 panel saw were not provided locks to isolate the panel saw with electrical and pneumatic sources of energy. b) On or about 09/15/2016 - Dato department, building #3, supervisors that had been authorized to lock and tagout equipment such as, but not limited to, the Selco WNT 600 panel saw were not provided locks and tags to isolate energy sources on the panel saw with electrical and pneumatic energy. Supervisors were going to the maintenance department to borrow locks when situations arose that required the supervisor to lock and tagout.
