29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):       On or about August 18, 2016, employees tasked with completing service and maintenance activities such as; but not limited to, clearing jams from the Post folder-gluer (machine #143) were exposed to machine hazards associated with the rotating parts and ingoing nip points. The employer failed to implement energy control application steps, and emergency stop control buttons (e-stops) and the Energy Safe Position (ESP) key were used in lieu of lock out [per the 1910.147(d)(2) requirement]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3), LOTO device application [(d)(4), dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6), were not implemented to protect employees from machine servicing hazards.   Prepare for shutdown [(d)(1)] - ensure authorized employee has knowledge of the type, magnitude, the hazards, and the method or means to control the energy;    Shutdown the machine or equipment [(d)(2)];    Machine isolation [(d)(3)] - Physically locate and operate all energy isolation devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s);   LOTO device application [(d)(4)] - Affix a lockout or tagout device to each energy isolating device;   Dissipation of residual energy [(d)(5)(i)] - relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy;   Verification of isolation [(d)(6)] - Verify that isolation and deenergization of the machine or equipment has been accomplished

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:       On or about August 06, 2016, guarding to protect employees from hazards created by rotating parts and/or ingoing nip points was not provided for employees tasked with operating the company's (Post) folder-gluing machinery, located in the Converting department. Employees were not protected from the following:     a. The end of the conveyor and the in-feed area of the strapper unit had a void of approximately 2.5 - 4 inches exposing to caught by and/or potential entanglement hazards.        b. Although the employer installed barrier gates around the Post folder-gluing machinery, it was inadequate and did not prevent entry into compression gates 1- 4.  The fencing interlocked and/or affixed to prevent entry thus potentially exposing employees to hazards created by rotating part.
