29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, and fittings shall also be effectively closed.  a. On September 14, 2016, the temporary electrical box had an unused opening near the built in receptacles, thereby exposing employees to electrical hazards.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  a. On September 14, 2016, employees were walking the top plate without fall protection, thereby exposing employees to a fall hazard of 11 feet.

On September 14, 2016, employees were performing work from the top step of a Werner six foot step ladder, thereby exposing the employee to a fall hazard of 6 feet.  b. On September 14, 2016, an employee working on the top plate used a six foot Werner step ladder to gain access to and from the top plate, thereby exposing the employee to a fall hazard of 11 feet.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.  a. On September 14, 2016, employees were working from the top step of a step ladder and using a step ladder as a means of access and egress from the top plate without training, thereby exposing employees to a fall hazard.
